Patient responsibilities

Provision of an efficient service is dependent on us working together with you, the patient. Please remember to cancel your appointment, giving 24 hours notice. This enables the appointment to be used by someone else. If you are late for an appointment, you may be asked to make another appointment.


Change of address

It is your responsibility to inform us of change of address. If you move and do not inform the practice,there is a danger you will be removed from our list by Health Authority procedures. You may phone, send or email your change of address details to us.  You can even do this by completing our Change Details form.

If you have a referral to a hospital waiting, you must inform them separately of your new address - failure to do so could delay and sometimes cancel a referral.  This is the most common delay when waiting for a referral date to come through.
