The personal information you give us will be used in your medical care. Some information will be passed to the Health Authority for administrative purposes.

Staff employed by the practice will have access to your medical records and other visiting clinicians such as the phlebotomist, midwife and health visitors would be able to access your medical records.

Everyone who may have access to medical records will have signed a confidentiality agreement. Information is never disclosed to a third party, including the University without your permission.

The only exception to this would be if it were believed that someone may come to serious harm if the information was not disclosed. You would be informed that confidentiality was to be broken
